Samantha Brooke Chismark, 13 years, of Fort Madison, Iowa died at the University of Iowa Hospitals and Clinics, Iowa City, Iowa. She was born February 20, 2001 at Iowa City, Iowa, a daughter of John and Melissa (Nehring) Chismark.

Samantha attended the Mt. Pleasant Christian School. She was working to become a youth minister and had already touch many lives. Her goal in life was to become a veterinarian.

She is survived by her parents: Melissa (Charles) Monroe, of Fort Madison, Iowa and John (Codi Filson) Chismark, of Burlington, Iowa; one brother, Branden Chismark, of Fort Madison, Iowa; two sisters, Britney Wittich, of Fort Madison, Iowa and Stephanie Chismark, of Fort Madison, Iowa; paternal grandmother, Lois Hadden, of Fort Madison, Iowa; two aunts, one uncle, and six cousins. She was preceded in death by her maternal grandparents, paternal grandfather, and an infant sister, Makiha.

Her body has been cremated. Her family will greet friends from 4:00 - 5:00 pm Thursday, June 19, 2014 at the Barr Memorial Chapel, Fort Madison. A Celebration of Life service will be held at 5:00 pm Thursday, June 19, 2014 at the Barr Memorial Chapel, Fort Madison, Iowa. Private family burial will be at a later date. A memorial has been established, contributions may be left at the Barr Memorial Chapel.
